Co-op Bank: Can my mother get pension?

This is regarding my Dad who passed away in 1995. He was working in a Co-Operative Bank at the time of his accidental death. It was a time of great sorrow to my family and my mother being not as educated to understand the processes involved to file for pension.

My Father’s details: Name: Ram Babu Inturi, Post: Bank Secretary ; Bank Location: Appikatla village, Bapatla Mandal, Guntur District, Andhra Pradesh, India

I would like some help in understand the following.

1.Was my father eligible for pension at the time of his death? I understand that in 1995 cooperative bank employee pension schemes was introduced in 1995.

2.Is there a way that my Mother can file for pension now?

3.If yes, what is the process to file for pension now?
